The Lung Allocation Score prioritizes lung transplantation candidates balancing waitlist-mortality and post-transplant survival. score groups sarcoidosis based on mean pulmonary artery pressure: ≤30 mmHg (Sarcoidosis A) are grouped with COPD >30 D) idiopathic fibrosis. We hypothesize that have a higher than other within their LAS grouping. This is retrospective cohort study of consecutive from the Scientific Registry Transplant Recipients database May 2005 to 2019. included ≥18 y/o diagnosed sarcoidosis, or IPF. Univariate, multivariate survival estimate analysis were performed. identified 385 A, 642 D, 7081 COPD, 10 639 IPF candidates. 17.3% 14.8% IPF, 14.3% 9.8% died awaiting transplant. Sarcoidosis A was an independent risk factor for waitlist-mortality. had lower waitlist probability compared COPD. D highest in first 60 days after listing. Based our results grouping allocation systems should be revised mitigate disparity.
